Stay Updated: Once you've signed up, we’ll send an email confirming your spot on the waitlist. We’ll update the website on the waitlist number that is next.

Authorization Requirement: In order to schedule an appointment, you must have an active authorization for Sparkle. Please ensure that your authorization is for Sparkle Where Our Voices Shine before submitting this form. Once appointments are available, we will not be able to hold spots for pending authorizations or to transfer referral.

Authorization Validity: It is important to verify that your authorization is still valid. Most authorizations are only valid for 6 months. You are responsible for confirming that your authorization is current. If your authorization expires before we can offer you an appointment, you will need to renew it with your insurance provider.
What to Expect While You're on the Waitlist

Estimated Wait Time: Depending on the demand, wait times can vary. We will do our best to get you an appointment as quickly as possible. You will be notified of any delays or sooner openings.

Telehealth Options: In some cases, telehealth appointments may be available while you wait for an in-person session. Let us know if you’re interested.
